The intent of this web site is assist educators, school personnel and families in determining which service will provide the most appropriate support to a student who is leaving the school system.
The process of preparing services for the needs of a student should begin at age 15. The Departments of Health, Family Service and Education have determined that the responsibility for identifying students, who will require special attention in transitioning to services outside the school system, lies with the school and that schools will start the transition process.
It is often very confusing for educators and families who may intersect with the adult services system infrequently to direct referral appropriately. The early involvement of potential service providers will allow the time necessary to determine service needs, and avoid any delay or last minute planning. The information in this web site should assist, or at least provide some contact names and numbers for seeking information.
